Ask the school what it needs
Contact the school office or attendance team to explain the absence and ask about its process. A routine absence, an examination and a request for special arrangements may involve different paperwork.
Before arranging a healthcare appointment, check whether the school has a form, requires a particular type of practitioner or needs evidence by a specific date. Share these instructions with the provider so they can explain what they are able to assess.
Prepare the details before the appointment
- The student’s name and the relevant dates of absence.
- The reason the school is requesting evidence.
- Any school-issued form or written instructions.
- The submission deadline and the school’s contact details.
If a child needs medical care, arrange that with an appropriate healthcare provider. Do not use an adult work-certificate application to request a child’s school document.
A parent’s work absence is a separate request
If you miss work because you are caring for someone else, you may be looking for evidence for your own carer’s leave, rather than evidence for a school absence. These are different purposes.
MyMedicalCert offers a carer’s leave assessment for eligible adults who live and work in Australia. It covers today only and costs $12.99 AUD, with a certificate emailed if approved. It does not produce a school attendance certificate for the child.

It costs $12.99 AUD, with a one-day sick leave or carer’s leave certificate emailed if approved. Payment does not guarantee approval. Check your workplace’s evidence requirements before applying.
